Pellagra
P ellagra is a d erm atitis resultin g from n iacin an d / or tryp top han e d eficien cy (in p erson s

Clinical features
C lassically, d isease of the „three D s‟: d erm atitis, d iarrhoea an d d em en tia.
– D ark red p laq ues, w ell d em arcated , sym m etric, located on exp osed areas of the bod y
(forehead , n eck , forearm s, legs). T he sk in becom es very scaly, p igm en ted , som etim es
w ith haem orrhagic bullae.
– G astroin testin al (glossitis, stom atitis an d d iarrhoea) an d n europ sychiatric sym p tom s
are seen in m ore serious form s.
Traitement
– nicotinamide P O
C hild ren an d ad ults: 3 0 0 to 5 0 0 m g/ d ay in 2 d ivid ed d oses, give w ith a d iet rich in
p rotein un til the p atien t is fully cured .
– In the even t of an ep id em ic of p ellagra, for exam p le in a refugee cam p , it is vital that
the food ration be m od ified (ad d groun d n uts or d ry vegetables) in ord er to m eet the
d aily req uirem en ts (ap p roxim ately 1 5 m g/ d ay for ad ults).
